Educational Health and Care Plan (EHCP) survey

The majority of children and young people with special educational needs or disabilities will have their needs met within local mainstream early years settings, schools or colleges. Some children and young people may require an EHC needs assessment for us to decide whether it is necessary for it to make provision in line with an EHCP.

The purpose of an EHCP is to make special educational provision to meet the special educational needs of the child or young person, to secure the best possible outcomes for them across education, health and social care and, as they get older, prepare them for adulthood

We want your feedback to help us to make the assessment process as straight forward as possible for parents and young people. Your feedback will also help us to make sure that the plans we write are clear, accurate and easy to understand.

Your feedback will be reviewed by the local area SEND board every month to help us to improve the way we deliver the EHCP process. We will publish all feedback provided on the local offer annually.
