Request an appointment to register a death

Use this form to request a phone appointment to register a death.

This form needs to be completed by the person registering the death (the 'informant').

Once you have submitted your request, the Registrar will contact you to complete the registration once they have confirmed receipt of the Doctor's and Coroner's documentation (if required). This will be done as soon as possible, but may take up to 5 working days.
